Your health. Your data. Your network.
Your record in your custody. Your care in coordination.
Health data wasn’t built to travel.
Your records live inside the system that made them. Switch providers — they stay behind. Cross a state line — they don’t follow. See a specialist — fill out the same forms, repeat the same history, repeat the same tests.
POHSN starts here: One whole record, in the patient’s custody, that grows with every visit.
Meet the App
Patients open it. Permissions move.
POHSN App is where the network lives in your hands. Your records, your demographics, your insurance — held in a wallet only you can open. Grant access at the moment of care. Revoke it the second you leave.
Meet the
Tap.
Consent you can see. At partner clinics, POHSN Tap sits at the entrance to every room. The patient taps their phone, says yes on screen and The Tap lights up - consent given to the right doctor, in the right room, for everyone to see. When the visit is over, one more tap ends access before they reach the hallway.
Meet the Point of Care.
At the point of care, the provider opens what the patient granted - and sees their standing directives first. When the visit ends, the doctor’s note comes back into the patient’s record before they leave the building. The next doctor they choose starts where this one finished - every doctor working from the same whole record, with the patient as the link between them. Access ends when the patient says. Every touch leaves a receipt.
Built for the patient layer.
Custody, not access. A portal gives you a window into one hospital’s record. Record apps gather your history and send it out. POHSN gives you the record itself - and brings every doctor’s note back into it. It lives with you on your phone, or locked under keys only you hold, and POHSN is built never to keep a copy it can read. Every grant has an end. Every view leaves a receipt. Every note comes home.
THE BET
Patient custody, Coordinated care.
For decades, healthcare has organized itself around institutions, and patients carried the gaps between them. Most tools pick a side — the hospital's or the patient's. POHSN doesn't. We bring patient and provider together through custody and coordination: the patient holds the record, and every doctor they choose works from it.
